Because your title proves that you are your vehicle’s legal owner, you should always keep it in a safe place. This document is required when selling or registering your car for the first time. Items Required to Get a Duplicate Louisiana Title
You must complete a Vehicle Application (Form DPSMV 1799) to obtain a duplicate title. You must also have your driver’s license or a valid photo I.D. and a copy of your Louisiana registration certificate.
All Louisiana title replacement application forms and affidavits must be notarized.

- License Plate Number
- Vehicle’s VIN, Year, Make, and Model
- Proof of Insurance
- Expiration Date of Registration Sticker
- If your vehicle has a lien, your lienholder or lender must complete and notarize an Affidavit of Non-Possession of Title by Lienholder (or duplicate title affidavit). If you are changing vehicle ownership at the time of application, you should also bring a document proving that the lien has been satisfied. (if applicable)
- If you’ve moved out of state, you must obtain a letter from the DMV in your new form confirming that your vehicle has not received a duplicate title. (if applicable)

To replace a title in person, you must:
