What will I need to get a duplicate title in Ohio?

To obtain a replacement title in Ohio, you must gather all of the necessary information and documents, which include the following:

  • A Completed Application for Certificate of Title to a Motor Vehicle (Form BMV 3774) | (Backup Link)
  • The Vehicle’s Year, Make & Model
  • Vehicle Identification Number (VIN) and/or Ohio Title Number
  • Vehicle Registration (for Proof of Ownership)
  • Acceptable Identification (such as a Driver’s License or an ID Card)
  • $15.00 Duplicate Title Fee
  • Lienholder Notation (if applicable)

How can I apply to get a duplicate title in Ohio?

In Ohio, you can apply for a duplicate title online, in person through the County Clerk of Courts Title Office, or by mail.

Apply Online

To apply for a replacement Ohio title online, you must already have an active title and have no liens against it.

NOTE: Only titled owners or those with authorization can request a replacement title.

Apply by Mail

For mail applications, include a filled out copy of the Certificate of Title to a Motor Vehicle (Form BMV 3774) as well as the $15.00 application fee. Make your check or money order out to the Ohio Bureau of Motor Vehicles.

Form BMV 3774 must include the mailing address where the new title should be sent, the requestor’s signature, valid notarization, and a lienholder notation (if applicable).

Apply in Person

How long does it take to get a replacement title in Ohio?

After submitting your application, you should expect to receive it within two to six weeks, though it may be sooner.

Out-of-state titleholders may be eligible for same-day service. Even if you are not an out-of-state titleholder, it is worth exploring whether same-day, over-the-counter printing is available in your area.
